What it is

GPs can refer patients with a chronic condition for allied health sessions under a Chronic Disease Management (CDM) plan, formerly known as an EPC plan, with a Medicare rebate attached. It's typically available if you have a chronic condition your GP has been managing for six months or more, things like diabetes, arthritis, heart conditions or persistent pain. Your GP makes the final call on whether you qualify, so check with them first.
